Purpose: Receiving your appointment request, contacting you and scheduling the appointment.

Transfer: Form submissions are delivered to the data controller’s e-mail through an e-mail delivery provider whose servers are located outside Turkey; your explicit consent is obtained for this transfer. Data is not shared with any other third party.

Retention: Appointment requests are deleted within a reasonable period after scheduling; medical records are kept for the periods required by health legislation.
